Payment Policy

Axis Endocrinology is a cash-pay practice and does not bill health insurance for physician services. All fees are due at the time of booking or enrollment.

Accepted payments: Credit card, debit card, HSA, FSA
Card on file: Required to book any appointment
Superbill: Provided upon request after each visit for out-of-network reimbursement. No guarantee of insurance reimbursement.
Medicare/Medicaid: Not billed for office visits. Patients may still use Medicare/Medicaid for labs, imaging, medications, and durable medical equipment (CGM, insulin pump, etc.).

New Patient Consultation

All new patients must complete a new patient consultation ($595) before enrolling in any package or membership. The consultation fee is non-refundable once the appointment has been completed.

À La Carte Visit Policy

À la carte visits are available to all patients on a per-visit basis with no commitment required. Payment is due at the time of booking.

Package Benefits

All packages and memberships (Foundation, Balance, and Rhythm) include the following benefits at no additional charge:

All packages must be paid in full at the time of purchase
Package visits are non-transferable and may not be shared with or gifted to another individual
Package visits are non-refundable once purchased
Unused visits that expire will not be refunded or credited
Re-enrollment fee of $100 applies to lapsed packages restarted after cancellation

Enrollment

Completion of a new patient consultation ($595) is required before membership activation
Membership begins on the date of first monthly payment
By enrolling, patients agree to a minimum commitment (6 months for monthly; automatic for prepay options)

Included Services

Each monthly membership period includes:

1 follow-up visit (45 min) — must be scheduled and used within the calendar month; unused visits do not roll over
All package benefits listed above

Visit Rollover Policy

Monthly visits do not roll over. If a patient does not schedule their monthly visit within the calendar month, that visit is forfeited. Patients are encouraged to schedule visits at the beginning of each month.

Cancellation Policy

Monthly option: patients may not cancel before completing the 6-month minimum commitment
Prepay options: non-refundable; no cancellation before term ends
After minimum period: cancellation requires 30 days written notice submitted via email
Membership fees are not prorated for partial months
Scheduled visits within the notice period will be honored
Re-enrollment fee of $100 applies if a patient wishes to rejoin after cancellation

Membership Pause

Membership may be paused once per year for up to 60 days for documented medical or personal emergencies, at the discretion of Dr. Behari. Pause requests must be submitted in writing via email.

Monthly Billing

Membership fee is billed automatically on the same date each month
Failed payments will result in a 5-day grace period, after which membership access will be paused until payment is resolved
HSA/FSA cards are accepted for monthly membership billing

Cancellation & No-Show Policy

Notice Requirement

All appointment cancellations and reschedules require a minimum of 48 hours advance notice prior to the scheduled appointment time.

Fee-for-Service Patients (À La Carte)

Cancellations or reschedules with less than 48 hours notice: 50% of visit fee charged to card on file
No-shows (failure to attend without notice): 50% of visit fee charged to card on file
Charges are non-refundable

Package & Membership Patients

Cancellations or reschedules with less than 48 hours notice: visit is forfeited from the package or membership
No-shows: visit is forfeited from the package or membership
No additional fee is charged — forfeiture of the visit is the only consequence

One-Time Courtesy Waiver

Each patient is entitled to one courtesy waiver of a late cancellation or no-show fee/forfeiture during their time as a patient of Axis Endocrinology. The waiver is applied automatically to the first offense. Subsequent late cancellations or no-shows will be charged or forfeited per the standard policy.

Extenuating Circumstances

Cancellation fees or forfeitures may be waived at the discretion of Dr. Behari in cases of documented medical emergency. Requests must be submitted in writing within 48 hours of the missed appointment.

Messaging & Between-Visit Communication

Secure messaging is available to all active package and membership holders
Messaging is intended for non-urgent clinical questions between visits
Response time: within 1–2 business days
Messaging is not appropriate for medical emergencies — patients experiencing a medical emergency should call 911 or go to the nearest emergency room
Complex questions requiring significant clinical judgment may be redirected to a scheduled visit

Refund Policy

New patient consultation fees are non-refundable once the appointment has been completed
Package fees are non-refundable once purchased
Prepaid membership fees are non-refundable for the prepaid term
Monthly membership fees are non-refundable for completed billing periods
Refund requests for extenuating circumstances will be considered on a case-by-case basis and must be submitted in writing

Policy Updates

Axis Endocrinology reserves the right to update pricing and policies. Current patients will be notified of material changes with a minimum of 30 days advance notice. Continued use of services constitutes acceptance of updated policies.
