Appointments are requested through our secure scheduling platform (Acuity Scheduling) and are subject to availability and confirmation.

Axis Endocrinology PLLC is based in Phoenix, Arizona and offers both in-person and virtual care options.

Payment Policy

Axis Endocrinology is a cash-pay practice and does not bill health insurance for physician services.

  • Payment is due at the time of booking or enrollment

  • Accepted forms of payment: credit card, debit card, HSA, and FSA

  • A valid card on file is required to schedule any appointment

  • A superbill can be provided upon request for potential out-of-network reimbursement (reimbursement is not guaranteed)

  • Medicare and Medicaid are not billed for office visits

Patients may still use Medicare or Medicaid for labs, imaging, medications, and durable medical equipment when applicable.
