Complete Your Intake
After booking, you’ll receive intake forms to complete before your visit.
Please take your time — the more Dr. Behari knows ahead of time, the more your visit can focus on understanding your case rather than gathering background.

Most visits take place via secure video using doxy.me — no app download required, just a link you can open from any device.
You must be physically located in Arizona at the time of your visit due to licensing requirements.

Lab orders are sent electronically to a major reference lab near you — including LabCorp and Sonora Quest. Lab fees are billed separately by the lab and are often covered by insurance. Results are sent directly to Axis for Dr. Behari to review and discuss with you.
Prescriptions are sent electronically to your preferred pharmacy. If you have questions about medication costs, we're happy to discuss affordable options during your visit.

We believe in transparent, straightforward pricing. New patient consultations are $595, and follow-up visits are $350. We also offer care packages for patients who want structured, ongoing support at a discounted rate. Visit our Pricing page for the full fee schedule and package details.

No membership required — you can book a single visit as needed, or choose a care package for structured support at a discounted rate.
For patients who need ongoing monthly care — active diabetes management, GLP-1 titration, perimenopause, or complex endocrine conditions — the Axis Rhythm Membership includes one follow-up visit per month, plus secure messaging, priority scheduling, lab result interpretation between visits, prior authorization handling, and more. Membership starts at $250/month with a 6-month minimum, with prepay options available at a discount. At $250/month versus $350 per à la carte follow-up visit, consistent members save $100 per visit.

Axis Endocrinology is a cash-pay practice and does not bill insurance directly. We provide a superbill after every visit — an itemized receipt with diagnosis and procedure codes that you can submit to your insurance for potential out-of-network reimbursement. Many patients with PPO plans recover a portion of their visit cost this way. We recommend calling your insurer before your appointment to understand your out-of-network benefits.

Yes — we recommend maintaining your health insurance coverage. Axis covers your endocrinology visits, but insurance remains important for medications, lab work, imaging, hospital care, and routine primary care. Visit fees do not include labs, imaging, or medications — those are billed separately by the respective providers. We provide a superbill after every visit that you can submit to your insurer for potential out-of-network reimbursement. If you need a copy sent directly to you, just let us know.
If you are currently uninsured, you are still welcome at Axis. We can work with you to keep lab and medication costs manageable and will discuss affordable options during your visit.

Yes. Axis Endocrinology visit fees are eligible medical expenses and can typically be paid using Health Savings Account (HSA) or Flexible Spending Account (FSA) funds. We recommend confirming with your plan administrator if you have any questions about eligibility.

No referral needed — you can book directly, and we typically offer same-week appointments. If you have a primary care physician, we're happy to coordinate with them and keep them in the loop about your care.

Not exactly. Concierge practices typically charge a membership fee on top of insurance billing. Axis uses a direct-pay model instead — one transparent fee, paid directly to your physician. What you see is what you pay.

Axis Endocrinology is currently licensed in Arizona. We see patients statewide via telehealth — including rural and underserved communities across the state. In-person visits are available in Phoenix on select days.

Yes. In-person visits are available in Phoenix on select days. In-person care is a great option for patients who prefer face-to-face visits or for certain treatments that benefit from an in-office appointment. All other care is available via secure telehealth statewide.

Yes. Every visit at Axis is with Dr. Behari. You will never be handed off to another provider without your knowledge and consent. Your care stays continuous and consistent — with a physician who knows your full history every time.

New patient consultations are 60 minutes. Follow-up visits are 30 minutes. We build longer visits into our model on purpose — because your health deserves time and attention.
